The useful test of ribociclib’s original proposition was what came with the disease-control evidence. FDA’s March 13, 2017 authorization covered postmenopausal women with HR-positive, HER2-negative advanced or metastatic breast cancer, with an aromatase inhibitor as initial endocrine-based therapy. This March 16 retrospective evaluates that defined regimen’s evidence and constraints, not a contest between competing brands. [209092Orig1s000ltr][209092s000lbl]
The March 2017 label reported an interim progression-free-survival advantage for ribociclib plus letrozole over placebo plus letrozole in MONALEESA-2. Median progression-free survival was not yet reached for the ribociclib group, against 14.7 months for the comparator. That supplied a benefit-side argument, but not an established overall-survival gain: those data were immature. Nor was placebo another CDK4/6 product. [209092s000lbl]
The same label described concentration-dependent QT prolongation. Its conditions included a QTcF below 450 milliseconds before starting, ECGs before treatment, around day 14 and at the second cycle’s start, and additional ECGs as clinically indicated. Electrolytes, liver-function tests and blood counts also required monitoring. The proposition was therefore not simply an oral medicine with a favorable efficacy result; it included clinically significant risk and ongoing assessment. [209092s000lbl]
FDA’s required alternative-dosing trial sharpened that trade-off: its aim was to reduce QT risk without compromising efficacy, not to announce that the problem had been solved. Read together, the letter and label support appraisal of disease control under those safety constraints. They do not establish an individual patient’s net benefit, superiority to a rival medicine, lower prices or better access. This is a historical assessment, not current treatment advice.
