Medicare’s payment reform does not give every local practice the same participation choice. Under CMS’s November 4, 2016 final rule, clinicians with at most $30,000 in Part B allowed charges or at most 100 Medicare patients are excluded by the low-volume threshold. CMS considered, but rejected, letting those excluded clinicians enter MIPS for a payment adjustment. [2016-25240][2016-10032]
That boundary matters to the incentive story. For eligible clinicians, the 2017 transition performance year offers different reporting paces tied to 2019 adjustments, while Advanced Alternative Payment Models form a separate avenue with qualifying conditions. The rule becomes effective January 1, 2017; it does not establish an immediate change in every practice’s revenue. [2016-25240][2016-10032]
The May 9 proposal supplies the earlier design logic: combine parts of existing programs, score four performance categories and use a different scoring standard for certain alternative-model participants. The final transition gives cost zero weight. Comparing them explains why a general promise to reward efficiency cannot substitute for checking which measures count initially. [2016-25240][2016-10032]
For competition among practice models, the useful question is which organizations can participate under which conditions, not whether reform automatically favors small businesses. This November 12 retrospective makes no claim about market shares or closures. Both sources come from the same CMS process. They show incentive architecture and entry boundaries, not measured competitive effects or guaranteed gains from switching arrangements. [2016-25240][2016-10032]
